SPORTS PHYSICALS & FORMS

All students participating in Lansing Athletics are required to submit a Physical, Concussion & Head Injury Release Form, and a Medical Consent Form. These forms need to be updated every year the student plans to participate in any sport.  Please contact Gary Mattingly, Activities Director, at Lansing High School if you have any questions regarding these forms.
