The Following is information REQUIRED for enrollment for Lansing Middle School:

• Actual Birth Certificate
• Record of Immunizations
• Proof of Residency – (Must be in Custodial-Residential Parent’s Name)

Telephone Statement, Gas/Electric Statement, Lan-Del Water Statement, Mortgage Statement,
Rental Agreement.

Other items that will be beneficial to the registration process are:

1. Student’s Social Security Number
2. Last Report Card
3. Copy of current I.E.P (Individualized Education Plan for Special Ed, Gifted-Idea, Learning
Disabilities, Emotional/Behavior disorders and Mentally Handicapped Students)
4. Any School Records from previous School
5. A copy or original of a valid Physical Exam for students interested in playing sports. May to May is what is considered valid. (Ie. A student wanting to participate in track had a physical given in March. That physical will expire in March making that student unable to participate in
track).
6. Any legal documentation stating Guardianship, Custody, Restraining Orders etc.
